Abstract
Long-term health conditions (LTHCs) such as diabetes, cardiovascular diseases, chronic respiratory illnesses, and cancer pose significant psychological, social, and physical challenges for affected adults. Beyond the medical management of these conditions, psychosocial support plays a critical role in enhancing resilience, promoting adaptive coping strategies, and improving quality of life. This paper explores the various psychosocial interventions, including counseling, peer support, cognitive-behavioral therapy (CBT), mindfulness practices, and digital support platforms, aimed at strengthening resilience among adults with chronic illnesses. Through literature review, case study analysis, and data collection, the research examines the effectiveness of these strategies in reducing psychological distress, enhancing treatment adherence, and fostering social connectedness. Data analysis and survey responses illustrate the positive impact of structured psychosocial support programs. Furthermore, the paper identifies barriers such as social stigma, lack of accessibility, and cultural constraints, while recommending best practices for implementation. The findings highlight the importance of integrating psychosocial care into routine health services and the pivotal role of multidisciplinary healthcare teams, particularly nurses and mental health professionals, in facilitating resiliencebuilding interventions
